Request for Occupational Health Records

All individuals have the right to know what information about them is held by the University. Under the terms of the General Data Protection Regulation 2018, individuals are allowed to ask the University for a copy of any personal data held about them as individuals. Individuals are entitled to all personal data that the University holds on computer, electronic or in manual record systems.

Accessing personal data in this way is known as making a subject access request (SAR).

Please note - for students requesting their Immunisations History:

• The request should be made by the individual.
• Please download and complete the Immunisation Request form and return to OHRecords@ljmu.ac.uk.
• We will process as soon as workload permits. This is usually within a fortnight.
