Request Immunisation History

All immunisation history that is provided by students prior to commencing their LJMU course will be held on an occupational health database. The occupational health nurses will also input the information of any immunisations you receive at the Occupational Health Unit.

If you require your immunisation history report, we can provide this to you free of charge.

Please be aware that the OH team have a maximum time of one month to complete the request.

To request your immunisation history, please complete all sections of the form below and return the completed form via email to ohadmin@ljmu.ac.uk or via post to 68 Mount Pleasant, Liverpool, L3 5UX.

Please specify on your request form if you would like the report sent to you via email or postal address.

When you receive your immunisation history report please keep this in a safe place and issue copies of the document rather than sending your original document.
