Accident Investigation

Despite improvements in technology and training, accidents involving ships and maritime infrastructure still occur. This course aims to help participants examine and identify the causal factors behind maritime incidents.

By understanding these factors, they will be able to develop recommendations that enhance safety at sea, protect the environment, and prevent similar incidents in future.

Courses based on IMO 3.11 “Marine Accident and Incident Investigation” are available on request.

Duration: Two or three days (based upon client requirements)

Prerequisites: Maritime Industry Experience

Assessment: Continuous monitoring of candidate performance and procedural understanding throughout the course with a final competence check conducted by an end of course written assessment.

Teaching: The candidates are provided with interactive classroom sessions with ongoing workshops/exercises and presentations.

Course overview

Investigating incidents through simulation

Simulation is a powerful method for reconstructing and analysing navigational incidents, which are often too complex and time-consuming for traditional investigation methods. It allows incidents to be recreated in a real-time simulated environment, making it possible to study key variables such as:

  • equipment failure
  • human error
  • vessel characteristics
  • operational procedures
  • environmental conditions

Benefits of a simulated approach

Using simulation makes the investigative process faster and more accurate. Analytical reports are produced in both digital and printed formats to support debriefing and review. Compared with conventional investigation techniques, simulation can significantly reduce both time and cost. 

Simulated reconstructions can also assist judicial and regulatory processes. Seeing an incident unfold dynamically helps non-specialists understand what happened and why. Multiple “what if” scenarios can be tested quickly, helping to identify the most likely causes. 

Clients’ operational procedures can be reviewed in light of investigation findings. Any necessary modifications or updates can then be introduced, followed by targeted training to confirm understanding and compliance.

Industry collaboration

The Centre works closely with the Maritime and Coastguard Agency (MCA), the Marine Accident Investigation Branch (MAIB), and maritime legal and surveying firms in the Merseyside area.
